Diabetes Outpatient Services at The Queen Elizabeth Hospital

The Central Adelaide Diabetes Service provides inpatient and outpatient services for patients living in the Central Adelaide Local Area Health Network with diabetes. Priority service is provided to:

  • type 1 diabetes, including transitioning from paediatric care
  • complex type 2 diabetes
  • secondary diabetes
  • cystic fibrosis related diabetes*
  • diabetes and renal / lung transplants
  • high risk pre-operative patients with sub-optimal glycaemic control.

*only provided at the Royal Adelaide Hospital.

The Central Adelaide Diabetes Service also provides diabetes education to patients listed above and for newly diagnosed type 2 diabetes. 

The Central Adelaide Diabetes Service also provides services to patients referred from rural and remote areas, including the Northern Territory, South West New South Wales and the Victorian Riverland.

Diabetes Priorities

Diabetes priorities are based on clinical urgency as displayed below:

Note: It is important to note these guidelines indicate what is clinically desirable, not what is always feasible in terms of delivery which is dependent on the size and staffing of the hospital department.

Immediate priority

Emergency care required.

Examples

  • Newly diagnosed type 1 diabetes
  • Diabetic ketoacidosis
  • Hyperglycaemic hyperosmolar coma

Referral process

Monday to Friday, 9.00 am to 5.00 pm must be discussed with the endocrine registrar on call.

After hours: refer directly to the Emergency Department.

Urgent priority

Examples

  • Type 1 with suboptimal blood glucose control
  • Major of problematic episodes(s) of hypoglycaemia
  • Transplant patients with suboptimal blood glucose control
  • High Risk Pre-Operative patients (internal referral)
  • Cystic Fibrosis Related Diabetes with suboptimal blood glucose control
  • Type 1 and Type 2 patients who are pregnant
  • Infected foot ulcer*
  • Acute Charcot Joint *
  • Cancer patients on steroids

*These clinics are held by Diabetes Centre at the RAH and Vascular Outpatients at the TQEH.

Referral process

Monday to Friday, 8.00 am to 4.00 pm. Discuss with Clinical Service Coordinator (RAH : (08) 8222 5111 / TQEH: (08) 8222 7030 or (08) 8222 7010) to obtain appropriate prioritisation and then fax referral letter marked URGENT to Referrals faxed to: RAH: (08) 8222 4265 or TQEH: (08) 8222 7188

Semi urgent priority

Condition is unlikely to require more complex care if assessment is delayed.

Examples

  • Cardiac Protocol (STEMI) review
  • Major or problematic episode(s) of hypoglycaemia
  • Poorly controlled type 2 diabetes with recent deterioration despite insulin therapy (typically HbA1c > 10%)
  • Non-infected foot ulcer in the patient with diabetes*

*These clinics are held by Diabetes Centre at the RAH and Vascular Outpatients at the TQEH.

Intermediate priority

Condition is unlikely to deteriorate quickly

Examples

  • Diabetes associated with recent diagnosis of significant micro or macro-vascular complications.
  • Diabetes associated with uncontrolled risk factors for CVD.
  • Type 2 with HbA1c greater than 9% on maximal oral therapy and/or symptoms of hyperglycaemia .

Non urgent priority

Low priority for appointment

Examples

  • Stable type 2 diabetes
  • Newly diagnosed type 2 diabetes*
  • Dietary advice for weight reduction, high cholesterol, hypertension or CVD in diabetic patients*
  • Impaired glucose tolerance or impaired fasting glucose*

* Patients with newly diagnosed type 2 diabetes, impaired glucose tolerance, or the “metabolic syndrome” should be managed by their general practitioner using local allied health and diabetes education services utilising a care plan.They should only be referred to a Central Adelaide Local Health Network facility when local services are not available or not practical.
