SA Community Care

SA Community Care provides short-term care in patients’ homes, after returning home from hospital or to help avoid a hospital stay when safe and appropriate.

Services supported through the SA Community Care program include nursing care, personal care assistance, allied health services and end of life care services.

The program is available at no charge to eligible public patients living in the Adelaide metropolitan area.

SA Community Care provides a range of services including wound care, medication visits, support with activities of daily living, end of life care, paediatric visits, continence management and more. All of these services are provided in the comfort of the patient’s own home, including residential aged care facilities.

A referral from a health professional is required for patients to access SA Community Care and all referrals must be directed to the Metropolitan Referral Unit.
