Northern Adelaide Geriatric Service (NAGS) at Lyell McEwin Hospital

NAGS aims to provide older people residing in northern Adelaide with high quality geriatric services in a variety of locations.

Service information

The ACE Unit is located at the Lyell McEwin Hospital and aims to provide short-term, streamlined, care for older people that are unwell.

Once treatment is completed, patients can be discharged to an appropriate destination through the guidance of a geriatrician led multi-disciplinary team consisting of medical, allied health and nursing staff. The service heavily focuses on optimising patient health caring for all aspects of the person’s needs in an inpatient setting.

Inclusion criteria

  • aged 85 years or older (or 50 years or older for Aboriginal or Torres Strait Islander people)
  • Clinical Frailty Score more than six with one or more of the following:
    • falls
    • delirium
    • non-operative fractures
    • acute illness not requiring care in another subspecialty
  • living with BPSD
    • tier 4-5

Exclusion criteria

  • older people with acute medical and surgical conditions who are requiring care under another specialty team

Patient is seen in the Emergency Department by a geriatric nurse consultant and is accepted by a geriatrician before being moved to the ward for specialist geriatric care.

  • Geriatrics in the Home (GITH) delivered in the home setting
  • Community Geriatric Evaluation and Management (CGEM) delivered in the home setting
  • Transition Care Program (TCP) delivered in the home or in a Residential Aged Care Home (RACH) setting
  • Care Awaiting Placement (CAP) delivered in a Residential Aged Care Home setting
  • Northern Reablement and Intermediate Care to Return Home (NRICH) delivered in a Residential Aged Care Home setting
  • Geriatric Evaluation and Management (GEM) Unit located at Modbury Hospital
  • Geriatric Rehabilitation (Geri IPR) Unit located at Modbury Hospital
