Vary an existing private health facility licence

This application is used to update an existing licence for administrative changes, such as:

  • Change of licensee name
  • Updates to governance structure
  • Modifications to leased areas
  • Other minor operational adjustments

Key timelines

  • Applications are typically processed within 6–8 weeks after all required documentation is received.
  • Approval timeframes may vary depending on the complexity of the application, the outcomes of inspections and any associated remedial actions.

The existing licence conditions remain in effect until the variation is approved.

Application process

  1. Prepare application – Gather all required documents to support the proposed variation
  2. Submit application – Complete and submit the online application form with supporting documentation
  3. Pay application fee – An invoice will be issued. Payment must be made before the application is reviewed
  4. Assessment and review – The application is checked for completeness. Additional documents may be requested if needed
  5. Site Inspection (if required) – The facility may be subject to inspection to verify compliance
  6. Variation approval – Once the assessment is complete, the variation is approved
  7. Licence issued – A new licence will be issued reflecting the approved changes

Prescribed Fees

Private Hospital licence

  • Variation of licence / conditions of licence - $2,837
  • Additional inspection of premises - $2,837

Private Day Procedure Centre licence

  • Variation of licence / conditions of licence - $1,702
  • Additional inspection of premises - $1,702

Invoices are issued after application submission. Payment options will be detailed on the invoice.

* If an inspection is required, the inspection fee is included in the original application fee. Additional inspections due to incomplete work or remedial actions will incur extra charges.
